Outward-swinging doors are more common and easier for a caregiver to open from outside in an emergency, but they require you to wait for the tub to drain before exiting, since water pressure holds the door shut while the tub is full. They also need clear floor space to open.
Inward-swinging doors can be opened at any water level and require less floor clearance, but are harder for a caregiver to access from outside. If you use a wheelchair or walker, outward-swing with adequate clearance in front of the tub is generally easier for assisted transfers.
Most walk-in tubs are designed to fit a standard 60-inch alcove, which is the same space a conventional tub occupies. Compact models are available for smaller bathrooms, and wider models exist for larger users or those who need more transfer space.
